HealthSpring offers Service Coordination for STAR+PLUS Members in an effort to work collaboratively with Providers and Members to assess Member health needs, create a care plan, organize services and monitor progress toward a Member’s individual health goals.  In addition, the Service Coordination team assists with long term services and supports such as adult foster care, adult day care, personal attendant services, minor home modifications, and home delivered meals.
